When: Monday, September 15 – Monday, November 24
Where: Hazelgrove, Adams Road, and Maddaugh Elementary School.
Time: 7:00PM – 8:30PM
Important Dates:
No games – October 19 (Thanksgiving)
Playoffs – Saturday, November 29
Cost: $575.00/Team

2) Fill out the captain’s form to complete registration, CAPTAIN’S FORM
3) Have teammates sign waiver form, WAIVER’S FORM
